Description
Essential job functions:
- Conducts home visits to assess family living environment and gather environmental samples and measurements to determine the need and develop environmental reports
- Ability to work with families who have multiple and serious problems
- Obtain medical history and housing status through health questionnaires
- Develop action plans for participants including goals, strengths, priorities, referrals and services to be provided
- Communicate with supervisor, team, clients and community partners on a consistent basis, ensuring success of referrals and partnerships
- Connect families with needed community resources, follow up with families and agencies to document the use and success of referrals
- Interact with community resources on behalf of clients
- Document all interactions into database and assign tier to home base findings
- Maintain inventory of supplies and materials
- Fulfill mandatory education requirements for obtaining and maintaining all credentials
- Ability to schedule appointments and adhere to deadlines
- Perform excellent customer service to customers, vendors, and staff by telephone, in person and/or in writing
- Adjust to frequent change in duties and procedures
- Be non-judgmental and objective in working with clients, vendors and coworkers
- Work independently and/or team setting
- Perform community outreach and facilitate educational workshops
- Ability to work effectively and efficiently in high stress situations
- Represent CAAGKC at special events, meetings and/or remote locations to collect and/or inform applicants of programs and services
- Inform participants of other CAAGKC programs and services they may qualify for
- Any other duties as assigned

Requirements
Minimum requirements:
- Bachelor degree in Social Work, Human Services, or a related field
- Experience providing housing, environmental health or public health assistance to individuals or families
- Ability to read, comprehend, and follow training manual guidelines and procedures
- Ability to work with the public
- Excellent listening, writing and assessment skills
- Experience with email, facsimile machine, scanner and copier
- Proficient in alpha-numeric filing, Microsoft Office products: word, excel, and outlook and ability to learn State agency database
- Reliable transportation, valid driver license and maintain active car insurance
- Some evenings and weekends required occasionally
Preferred qualifications:
- Health Education Specialist or Health Home Specialist Credential
- Experience with home maintenance a plus
- Problem solving skills
- Experience working with contractors
- Community health worker experience

Summary
Community Action Agency of Greater Kansas City is a nonprofit organization providing free programs and services to low-income residents in Clay, Jackson and Platte counties in Missouri.
A successful Healthy Homes Case Manager candidate has a bachelor degree in Social Work or related field (Psychology, Public Health, etc) with experience providing housing, environmental health or public health assistance, linkage to community resources, screening clients, and writing skills (case notes). This candidate will conduct detailed home visits to assess the family living environment to determine the need and develop environmental reports that identify safety hazards and conditions that affect the health and quality of life for individuals and families. After the assessment, education, cleaning kits, and vendor services will be coordinated and provided by the Case Manager. Uniforms, training, and equipment is provided.
